For your convenience, you can open and fill out the forms online and email us or print them out and bring them with you on your first visit.

What to Expect on Your First Visit

Once you arrive at our office, one of our receptionists will help you complete any necessary forms as well as answer any questions you may have about your insurance. Most patients see our dentist first, but you can have the option to be scheduled with one of our hygienists as well. You’ll receive a comprehensive examination, oral cancer screening, necessary X-rays, necessary intraoral images, and a cleaning if needed.

Amenities to Help You Feel at Home:

At North Woodbridge Dental, we have implemented a few simple conveniences to help you feel comfortable during your visit and also during your treatment. These include:

  • Games, coloring books and toys for your children to play with

Insurance Information

We are happy to file dental insurance claims on your behalf, as well as provide you with an estimate of your insurance coverage as it relates to treatment needs. All patients will receive an itemized treatment plan, so there are no surprises when it comes to expenses related to their care.

If you have Canadian Dental Care Plan, make sure to register online at CDCP member portal, and bring:

Your CDCP member card
A piece of ID
Your welcome letter that includes your benefit effective date

Payment Options and Financing

Our office accepts cash, Visa, and MasterCard. Patients may also choose to finance all or a portion of their dental treatment through HealthSmart. Applications are available at our front desk.
